We will go over your labs, bloodwork, records and order them in office during your visit if need be.
We will go over your initial plan - what's working, what's not, change medications or dosage depending on the individual, and continue to plan maintenance tasks while you're working towards your weight loss goal.

SUPERBILL AVAILABLE UPON REQUEST: A superbill is a summary of your visit that you can submit to your insurance company to request reimbursement directly to you if eligible through your plan. Contact your insurance provider to learn if you have out-of-network benefits for HRT. Medicare recipients do not have out of network benefits, thus cannot submit for reimbursement for the cost of their visit. If you have medicare or do not have insurance, please ask us about the Patience Assistance Program.
