Chronic Weight Management Appointments:
Initial One-Hour Consultation - 60 min
Your first initial visit will last around an hour normally. This visit will include a comprehensive weight evaluation, medical evaluation, individualized plan with nutrition and activity included, and a prescription for medication if needed.
We will go over your labs, bloodwork, records and order them in office during your visit if need be.
Follow-Up Visit Appointment - 20 min
We will go over your initial plan - what's working, what's not, change medications or dosage depending on the individual, and continue to plan maintenance tasks while you're working towards your weight loss goal.
ACCORDING TO THE FLORIDA BOARD OF MEDICINE, WE ARE REQUIRED BY LAW TO SEE YOU AT LEAST EVERY 3 MONTHS.

SUPERBILL AVAILABLE UPON REQUEST: A superbill is a summary of your visit that you can submit to your insurance company to request reimbursement directly to you if eligible through your plan. Contact your insurance provider to learn if you have out-of-network benefits for HRT. Medicare recipients do not have out of network benefits, thus cannot submit for reimbursement for the cost of their visit. If you have medicare or do not have insurance, please ask us about the Patience Assistance Program.
Direct Primary Care Appointments:
At Inspired Health & Wellness, we embrace the Direct PRimary Care (DPC) model to provide you with personalized, accessible, and affordable healthcare. Our focus is on fostering a strong patient-provider relationship and delivering comprehensive care without the interference of insurance companies. You will receive personalized attention, direct access to your provider, transparent and affordable pricing, and comprehensive care.
Initial Enrollment & Assessment Fee
This is a one time fee and during the initial visit, we will have you sign a consent form in order for us to get your past medical records. We will order labs if necessary and go over your history, etc. This appointment is 60 minutes.
monthly subscription
This is a monthly, recurring fee but includes all visits, sick visits, telemedicine visits, etc. The prices listed are based on a patient's age.
Hormone replacement therapy
At Inspired Health & Wellness, we treat the individual, not just lab values. Donna is an HRT expert who partners with her patients to explore different options for optimizing hormones based on their individual health history, symptoms, and labs.
Initial Enrollment & Assessment Fee
This is a one time fee and during the initial visit, we will assess symptoms, order and review labs, and discuss treatment plan options to optimize your hormones, energy, libido, muscle mass, and feeling of wellbeing. Labs are included. This appointment is 45 minutes.
Pellet Insertion for Males
This treatment will last 5-6 months and the appointment is 30 minutes.
Male Non-Pellet / HRT Follow Up
This appointment is 20 minutes.
Pellet Insertion for modified males
This treatment will last 3-4 months and the appointment is 30 minutes.
Pellet Insertion for females

Functional Medicine Appointments:
Tara Hall is a wonderful addition to the Inspired Health & Wellness team, focusing on an individualized, holistic approach to health. Tara offers Functional Medicine testing including Microbiome (Digestive Health) kit, Hair Mineral analysis, Omega 3 Ratio testing, and live blood analysis.
Functional Medicine Initial Appointment
In this 60 minute appointment, Tara will review your health history, lifestyle habits, symptoms, and personal health goals. She can also review your labs, any functional medicine testing, and provide a free live blood analysis.
